By attending a Web Cast and completing the Evaluation Form below, you can register for continuing education units (CEUs). Registering for CEUs is easy!

• Download the applicable Evaluation Form noted below (PDF format)
• Complete the form by supplying the required contact information and answering the questions contained in the Evaluation Form
• Fax (612-624-2750) or send the completed Evaluation form to:

• Once your completed Evaluation Form has been received and reviewed, we will send you a certificate of completion and a form from the National Council for Interior Design Qualification (NCIDQ) to register the CEUs.
• Send the completed NCIDQ Continuing Education Form with the appropriate fee to NCIDQ.

That's it! If you wish to register your continuing education experience with another organization or entity, please feel free to download the Evaluation Form, complete it, and send it to that organization or entity.
